Full Job Description
Support the overall organization of health services, including case management, research and support for the implementation of care plans to promote the effective use of health services. Promotes/supports the performance of quality medical services. It is responsible for the initial investigation and monitoring of the case management service. Assess patients using policies and business processes to target interventions to identify required medical services, refer patients to medical staff, and organize required services as a benefit plan. Monitors pending cases for release dates and decisions and informs Senior CM about upcoming releases.
Organizes and organizes the delivery of health care services under the direction of a Nurse, LCSW or Health Care Manager in the best conditions and at the best cost by identifying opportunities for the patient to use service providers and participating employees. Promotes communication, both internally and externally, to improve the efficiency of health care services (eg, health care providers and health care teams, respectively). Provides support services to team members by answering phone calls, taking messages, analyzing information and helping to resolve problems. Ensures compliance with PM policies and management standards. Maintains accurate and complete documentation of required information that meets risk management, regulatory and accreditation requirements. Maintains the confidentiality of member information and adheres to the company’s privacy policy. Support the management of case management and quality control processes in accordance with various laws and regulations, while complying with company rules and procedures.
Permission Required:
– Minimum of 2 years of experience in customer service, preferably in the healthcare sector.
– Two years of experience in managed care or human health
Eligibility Of Candidates:
– Two years working in health field preferred. – Medicare experience preferred.
– Bilingual – English/Spanish preferred.
