Full Job Description

Our Caregivers are those who support those who cannot support themselves. They are responsible for evaluating, planning, implementing and coordinating all case management services with members to assess the member’s medical needs to support the member’s well-being.

The basics:

• Develops proactive care plans to address identified issues to improve short- and long-term outcomes and opportunities to improve member well-being.
• Uses clinical tools and information/data analysis to conduct needs and benefits assessments for members. • Engages in clinical judgment to implement strategies designed to reduce risks and barriers and address critical health and social indicators affecting care planning.
• Performs research that considers information from various sources, such as statements, to address all conditions, including chronic diseases and many diagnoses that affect work. • Uses a comprehensive approach to determine the need for input from clinical resources and other members of the interdisciplinary team.
• Collaborate with the supervisor and other important people in the member’s health care to overcome obstacles to achieve goals and objectives, presenting issues in various hearings.
• Utilizes case management procedures in accordance with applicable laws and regulations. Use motivational interviewing skills to ensure maximum team involvement and determine their health status and health needs based on key questions and conversations.

Permission Required:

– Must have an active, unrestricted RN license in the state of KS or hold an active license in the state of residence with a multi-state/compact license.
– 3+ years of practical clinical experience as a registered nurse
– 2+ years of experience in case management, discharge planning and/or home health care planning. – 1 year and experience works in the number of adults

– Effective and efficient computer skills, including navigating multiple systems and applications
– Be willing and able to obtain a multi-state RN license if required, to be issued by the company. Education
– Associate degree required
– Bachelor’s degree preferred
